After common cold, if anything takes your work time, then it is low back pain. A research conducted in the recent times has shown that sixty five to eighty five percent of United States population is affected by this at some point in their life. The cause behind this is almost same like improper posture or poor lifting technique and in some cases bad back habits. Disc problem which is brought by wear and tear or age is the other most common problem. Causes may be like infections, fractures like osteoporotic and even tumors.

To ease the back pain and sciatica this new technology will be used for disc problems. About 7 million back pains are associated to disc problems in the United States. Since the technologies which are newly invented is unproven, it is advised to use the dose with caution. A contradiction arises at a point, that, disc problem treatments are invasive surgeries. So the newly adapted technologies must be tired as they guarantee a less invasive treatment.

Duty of spinal disc

The main duty of the disc is to absorb the shock in between the vertebrae. The discs are fibrous and tough. The annulus or the outer shelled disc is filled with the nucleus commonly called the gel. If it is a healthy back, the disc’s duty is to allow spine to be more flexible. The nucleus of the disc may bulge out and may sometimes herniate by the walls of the annulus leading to the annulus degeneration. This can happen even because of the time and trauma.

Injuries like the annulus and inter vertebral disc can only be verified with a CT scan or MRI. An interesting fact is that, sometimes a patient who has undergone many such tests is shown such abnormalities but they have never complained of back pain. The reason for so is yet to be understood. One thing is for sure that the source of pain is a degenerated disc. If the spinal’s root of nerve is pressed by the bulging disc, the pain basically radiates and is the cause of sciatica.

A concept of color drawing is done for disc problems which are labeled under normal degenerated bulging leading to the formation of herniated thinning osteophyte. The treatment options till now are very limited. The muscles can be eased by means of physical therapy. It is seen that the painful muscle is struggling to cope up with the spine problem. Physical therapy tries its level best to prevent abnormal stress on spine. The inflammation areas can be reduced by injecting Epidural steroid injections, but it is seen that the pain comes back if the problem underlying is severe. If it is an acute problem, surgery is the only option left. In surgery the vertebrae is fused to remove the pressure from the disc and in some cases to remove some parts of the disc’s.

As of now there are two unproven treatment technique’s available. Radio frequency Discal Nucleoplasty or in short Coblation Nucleoplasty and IDE the ways to treat Sciatica and back pain. IDET stands for Intradiscal electrothermoplasty.

Intradiscal electrothermoplasty

Here a needle is being inserted into the disc. The complete guidance is done by the x-ray machine. A wire is now being introduced or being threaded via needle to the disc. The threading is done till the wire has reached the inner walls of annulus. To destroy the fibers which have grown in the cracks the wire is heated.

This heat will melt the annulus partially thereby triggering the body for the generation of new protein fibers. The spine journal has published an article about the study of fifty three patients who were suffering from back pain. The success rate of Intradiscal electrothermoplasty is found to be as high as sixty percent and as low as twenty three percent.

Radio frequency Discal Nucleoplasty or Coblation Nucleoplasty

Nucleoplasty is the newest concept. It has now being available in the market for only a few months. As it was done in the case of Intradiscal electrothermoplasty, a needle is being inserted in the disc. Here the only difference is the use if radio frequencies probe instead of the heated wire. The radio frequency probe is inserted via needle to the disc. The duty of the probe is to break up the molecular bonds formed by the gel in the nucleus by the use of energy drawn from the highly focused plasma field. Here some part of the nucleus is also vaporized. About twenty percent of the nucleus is removed to reduce the pressure in the disc and the nerves surrounding it. It is seen that sciatica type pain is reduced in this technique rather than IDET, because here the pressure is much released from the nerve root. Relative low temperature gives birth to high energy plasma field leading to the minimization of dangerous surrounding tissue.

The new technique seems interesting as treatment or diagnose of low back pain along with sciatica without trauma can be achieved. The risk of surgery is greatly reduced. The disadvantage which goes with this is the method is still unproven. It is expected that vide researches many more tools will come which will help to get rid of such debilitating problems.
